LPN - Clinical Care Coordinator II

The Clinical Care Coordinator II provides patient care tailored to specific age groups and supports management of patient services in a physician office setting under direct supervision.
Qualifications include: Basic Life Support/CPR certification within 30 days of hire, and graduation from an accredited nursing school or diploma program.
Preferred experience: at least one year in outpatient clinical practice, familiarity with electronic practice management and electronic health records, and proficiency in Microsoft Office Suite.
Licensing: Must hold a valid FL Licensed Practical Nurse (LPN) license.
All care providers for vulnerable populations undergo background screening through the Florida Care Provider Background Screening Clearinghouse to ensure safety and compliance.
