Key Responsibilities
- Conduct initial and ongoing patient assessments to determine care and discharge needs
- Develop focused discharge plans and collaborate with healthcare providers to execute them
- Maintain current knowledge of healthcare trends, regulations, and payer policies
- Registered Nurse license with current Maryland eligibility; BSN preferred, ADN required
- 3-5 years of inpatient case management or discharge planning experience
- Knowledge of healthcare theory and practice within a specialized field
- Case management certification preferred within 3-5 years of hire
