Role Overview: As an LPN Medication Nurse, you will provide licensed nursing support by administering medications, monitoring resident health, and maintaining accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real time
• Monitor symptoms, report adverse reactions or refusals, and support care plan updates
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and ensure compliance standards
  
  

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
