Under Medical Director supervision, you will perform physical assessments, develop care plans, evaluate health complaints, and deliver appropriate treatments. You will also educate participants and caregivers, collaborating with an interdisciplinary team to ensure comprehensive, person-centered care.

Key Responsibilities Include
- Conducting assessments and physical examinations of new enrollees and semi-annual re-evaluations
- Providing medical treatment and coordinating care with primary care physicians and team members
- Overseeing residents in skilled nursing facilities and participating in discharge planning
- Developing and implementing individualized care plans
- Licensed Registered Nurse and Certified Nurse Practitioner
- Graduate of a professional nursing school
- Minimum one year of experience working with frail and elderly populations
- Geriatric Nurse Practitioner certification with prescriptive authority
- Two or more years of geriatric clinical experience
