Role Overview: This Occupational Therapist (OT) plans, organizes, and delivers OT services as part of an interdisciplinary team for participants in a comprehensive care program focused on promoting independence and well-being in seniors.

- Conduct initial and ongoing assessments of participants' functional abilities, home environments, and rehabilitation needs.
- Recommend and coordinate adaptive equipment, home modifications, and therapy interventions to ensure safety and independence.
- Develop, implement, and adjust individualized treatment plans based on participant progress and team input.
- Deliver therapy across various settings including centers, clinics, homes, and contracted facilities.
- Educate participants and caregivers on equipment use, safety strategies, and activities of daily living (ADL) support.
- Maintain accurate documentation in compliance with organizational and regulatory standards.
- Participate in daily interdisciplinary team meetings for comprehensive care planning and coordination.
- Supervise and mentor Certified Occupational Therapy Assistants (COTAs) and train staff on safe OT practices.
- Support quality improvement, utilization review, and ensure compliance with professional standards.
- Perform integrated IDT functions, potentially including roles related to home care coordination, based on licensure and experience.
- Master’s degree in Occupational Therapy from an accredited institution.
- Current state licensure to practice occupational therapy.
- At least 2 years of experience working within an interdisciplinary team; experience with elderly or frail populations preferred.
- Competency in OT practices and ability to meet position-specific requirements.
- Reliable transportation, valid driver’s license, and up-to-date immunizations and background checks.
