Provides comprehensive clinical social work services in primary care or home health settings, offering support, consultation, and care coordination. Evaluates patients' psychosocial assets and deficits to promote independence, collaborates with care teams to develop care plans, and delivers short-term counseling, teaching, and resource brokering.
Responsibilities Include
- Conducting psychosocial assessments and delivering interventions such as individual or family therapy, advocacy, referrals, group work, and long-term care planning.
- Collaborating with multidisciplinary teams, participating in care and family conferences, and communicating patient needs to healthcare providers.
- Assessing patient and family needs, monitoring ongoing response, and adjusting care plans accordingly.
- Minimum of one year of social work experience (waived for current employees per agreement).
- Master's degree in Behavioral Science or Social Work.
- Licensed Independent Clinical Social Worker, Mental Health Counselor, Licensed Advanced Social Worker, or Licensed Marriage and Family Therapist in Washington.
