Position Overview

Seeking a Physical Therapy Assistant to deliver skilled education, treatment, and interventions under the physician's plan of care in home health settings. Collaborate with interdisciplinary teams, providing updates to optimize patient outcomes.

Key Responsibilities

  • Provide direct physical therapy following directions from the physical therapist and physician.
  • Communicate regularly with patients and caregivers about the care plan.
  • Deliver treatments, procedures, and skilled therapy services ensuring patient safety.
  • Observe and report patient conditions, reactions, and progress to supervisors and physicians.
  • Assist in pain relief and function restoration to maximize performance.
  • Educate patients and caregivers on the safe use of prosthetic, orthotic, and assistive devices.
  • Collaborate with other disciplines on patient care plans.
  • Plan and communicate discharge instructions and prepare for patient discharge.
  • Participate in in-service training and adhere to ethical standards of the American Physical Therapy Association.
  • Maintain confidentiality, document clinical notes promptly, and ensure continuity of care.
  • Ensure timely submission of credentials and maintain professional conduct.

Qualifications & Skills

  • Graduate of an accredited Physical Therapist Assistant program.
  • Current, unrestricted state license as a Physical Therapy Assistant.
  • Valid CPR certification.
