Seeking a Full-Time Physical Therapist (PT) to provide home health care services primarily in South/Central Hillsborough County. The role involves evaluating patients' mobility, strength, and function through assessments, and developing personalized plans of care with clear goals and outcomes. The PT administers and interprets tests, delivers prescribed therapies, documents progress, and adjusts treatment as needed. Responsibilities include discharge planning, patient and caregiver education, coordinating with healthcare teams, supervising personnel (PTA, HHA), and completing OASIS assessments where permitted. The role requires minimal supervision, adherence to policies, and maintaining effective communication with staff, families, and other providers.

Qualifications
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state license as a PT
- Valid CPR certification
