Responsibilities

• Deliver skilled nursing services in patients’ homes and community settings.
• Perform medication administration, wound care, and monitor vital signs and clinical status.
• Follow individualized care plans and document all care in electronic records.

Qualifications & Skills

• Valid LPN license.
