Role: In Home Healthcare RN

Responsibilities

• Deliver compassionate, skilled nursing care to patients in their homes
• Perform comprehensive assessments and administer medications and treatments
• Develop individualized care plans in collaboration with physicians and interdisciplinary team
• Educate patients and families on disease management, safety, and self-care
• Document care accurately in electronic medical records
