Join a compassionate hospice team as a full-time On Call Registered Nurse (RN).
Deliver personalized end-of-life care, focusing on symptom control, pain management, and patient dignity.
Responsibilities include conducting home visits with empathy, assessing patient and family needs, educating caregivers, mentoring team members, collaborating with interdisciplinary staff, administering medications, managing symptoms, and participating in on-call rotations to ensure timely support.
Maintain accurate documentation, ensure compliance with certification requirements, and provide seamless care transitions.
Qualifications: Active RN license, current CPR, valid driver’s license, and reliable transportation.
Preferred: at least 1 year of hospice, home health, med-surge, or ER experience.
