Position: RN Care Manager

Location: Arlington Heights, IL

Hours: Full-time, Monday-Friday, 8:00 a.m. - 4:30 p.m., with rotating weekends and holiday coverage

Responsibilities Include

• Conduct comprehensive assessments of patients' medical, social, and psychological needs.
• Develop and coordinate personalized care plans in collaboration with healthcare providers.
• Identify barriers to care, facilitate referrals, and coordinate services such as home healthcare and specialist visits.
• Serve as a liaison between patients, families, providers, and community resources for seamless care delivery.
• Monitor patient progress, manage transitions of care, and ensure timely treatments.
• Educate patients and families on diagnoses, treatment options, and self-management strategies.
• Maintain accurate electronic health records and prepare reports on patient outcomes.
• Participate in quality improvement initiatives and ensure compliance with healthcare regulations, including HIPAA.
  
  

Qualifications & Skills:

• Bachelor’s Degree in Nursing required; Master’s preferred.
• Minimum of 2 years clinical nursing experience, preferably in acute or hospital settings.
• Current RN license in Illinois; CCM certification preferred.
• CPR/BLS certification from the American Heart Association.
• Strong assessment, communication, collaboration, and organizational skills.
