Role Overview: The Home Infusion Clinical Liaison supports the growth of home infusion services by building and maintaining positive referral relationships with discharge planners, facility staff, physicians, and patients.

Key Responsibilities: Assess suitability for home infusion, facilitate care transitions, ensure proper documentation, provide consultative education to providers and families, and serve as a clinical resource.

Qualifications: Licensure as an Oregon Registered Nurse, BLS certification, and at least one year of clinical experience. Preferred qualifications include a Bachelor's in Nursing, sales or marketing experience, healthcare industry background, prior medical management success, and EMR proficiency.
