Position Overview

This role involves transforming patient care through assessment, advocacy, and resource coordination to improve access and reduce barriers. The RN Nurse Navigator provides patient-centered support across diverse age groups, ensuring adherence to departmental policies and promoting quality service.

Key Responsibilities

• Facilitate access to culturally competent medical services by eliminating barriers and assisting with community resources and financial aid.
• Collaborate with healthcare providers, multidisciplinary teams, and community resources to meet treatment goals.
• Recognize and address problems, document all encounters accurately, and report relevant statistics.
• Participate in meetings, committees, and professional development activities to enhance growth.
• Ensure patient safety by following protocols, reporting concerns, and encouraging patient engagement in care.
• Assist with tests, procedures, medication administration, and emergency responses as needed.
  
  

Qualifications And Skills

• Graduate of an accredited nursing program; BSN or related bachelor's degree preferred.
• Minimum of 1 year of related nursing experience.
• Current CPR/BLS certification and RN licensure in applicable state.
• Strong skills in communication, organization, problem-solving, critical thinking, collaboration, and multitasking.
• Ability to deliver safe direct patient care across age groups and work in a sedentary, potentially high-risk environment.
